Summit County Birth Certificate and Name Change Legal MEGA Clinic

Introducing the state’s next Mega Birth Certificate and Name Change Clinic where attorneys will be available to answer your questions, notaries will be standing by to witness signatures, and a sitting magistrate will be able to process filings and issue orders on site!

When: Friday, Janury 24th, 10:00am until 3:00pm!

Where: Summit County Probate Court, 209 S. High St., Akron.

Trained volunteer attorneys will be available to help you complete forms and discuss your questions concerning legal name change, birth certificate correction, and more from
10:00am until 3:00pm!

Notaries will be available! Probate magistrates will also be able to process filings and issue orders on site!

There are costs associated with parking and legal filing fees.

Financial assistance will be available for those who qualify. Applications will be taken on site.

What to bring:
Government-issued photo ID
Copy of birth certificate
Cash, check, or money order for the filing fee(s)

Who is eligible:
For name change:
Summit County resident for at least 60 days

For gender marker correction:
Proof that you were born in Summit, in some instances a long form birth certificate may be needed.

For Ohio birth certificate correction:
Summit County resident, OR born in Summit County

*Folks should bring proof of residency if their ID is new (issued within 59 days) or doesn’t list an address in located in Summit County.
